Data controller

Data processed
Appointment and contact forms process your name, phone number, email address and any notes you send. If you share health-related information in a form, it is used only to respond to your enquiry and to plan your appointment.

Health data are special-category personal data. They are processed on the basis of your explicit consent or for the provision of assessment, psychotherapy and care services, by persons under a duty of confidentiality and only as far as your request requires.
Transfers
Your data are not sold to third parties. Where legally required they may be shared with competent authorities, and where necessary for the service they may be shared with laboratories, hospitals or IT infrastructure providers solely for the related purpose. Website hosting may be located abroad; in that case transfers are limited to the safeguards provided by the Law.
Your rights
Under the Law you have the right to learn whether your data are processed, to access processed data, to request correction, erasure or restriction of processing, to withdraw consent you have given, and to apply to the Personal Data Protection Board. You may submit requests via the contact form or at info@huseyindirek.com.
Retention and cookies
Form records are kept until the request is completed and for any statutory retention periods; they are then deleted or anonymised. The site uses only an essential session cookie for security and form validation.
